As the High-Value Network Associate Director, you will be responsible for ensuring performance outcomes, managing relationships, and providing ongoing support to organizations in the continuum of care. Your goal will be to foster the right care at the right place with optimal transitions for the Tandigm population.
You will guide facility and agency leadership in their transition from fee-for-service to value-based care as a population health subject matter expert.
