Starting annual salary $48,000
Working Schedule: Monday-Friday 9am-5pm
The Urgent Crisis Center is a walk-in, police or ambulance psychiatric crisis stabilization program designed to triage, stabilize, and assess youth who are experiencing psychiatric crisis, and then link to a lower level of care.
This program operates 24/7, 365 days of the year.
JOB SUMMARY
The Discharge Planner participates in an interdisciplinary team and is responsible for implementing the patient's discharge plan as detailed by the patient's treatment plan.
This position provides the patient and caregiver with a written, comprehensive discharge plan detailing the next phase of treatment; this should include the name of the program, contact person and telephone number. When applicable, this should also include the date of the appointment. They review and discuss the plan with the patient and caregiver to ensure everyone understands and agrees to follow it.
Even after the patient leaves the Urgent Crisis Center, the discharge planner remains available to answer any questions or provide new referrals if necessary.
KEY RESPONSIBILITIES
Facilitates discharge plan through an interdisciplinary team process in conjunction with the family, psychiatry, nursing and clinical staff.
Develops and maintains collaborative relationships with other community agencies and organizations in consultation with supervisor, in order to provide comprehensive and effective treatment services for clients (i.e. schools, courts, hospitals, etc.)
Completes all referrals and linkage to the identified treatment services
Assist families with identifying basic needs
Compile and update information on resources in the local area for the team.
Along with clinical staff, will provide aftercare phone calls within 24 hours of discharge, then daily up until start of next treatment provider.
Attend and participate in weekly rounds
Work as a member of an integrated system of care, in collaboration with parents, DCF and other community providers to supportive family.
Experience and Skills
EDUCATION
Bachelor's degree in social work or an equivalent human services field
EXPERIENCE Experience working with children in a healthcare or residential setting (preferred)
Knowledge of community resources
