In this
key role, you will be responsible for rotating
shifts and areas with a primary focus in oncology and CAR-T program patients.
You will preparation and dispense drugs in all inpatient settings; maintain and
review patient medication profiles; review and verify physician medication
orders and patient medical record. You will interact with healthcare
professionals and providers to help promote safe medication use, prevent
inappropriate or misuse of medications; provide clinical pharmacy services
(e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN
per pharmacy, clinical protocols per pharmacy and pharmacist interventions.
When assigned to decentralized duties in patient care and Oncology units, you
will be physically located in the patient care areas and primarily responsible
for verification of new admission medication orders, medication history taking
and reconciliation of high risk patients upon admission, and rounding with
MD/RNs beginning on admission to provide effective medication therapy during
the patient's stay and early planning for discharge.

Varied shifts, depending on department needs, including weekends and holidays

Salary Range: $83.37 - $97.65 (Union represented, step based salary depending
on years of experience)

Qualifications We're
seeking a dedicated clinical professional with:
Pharm D.
degree and current CA Pharmacy license (required).
ASHP-accredited
residency (preferred).
Completion
of PGY1 Pharmacy Residency or equivalent 3-5 years experience in inpatient
hospital pharmacy setting and/or in decentralized patient care pharmacy setting
Prefer
clinical background/residency in oncology and/or Board Certification in
Oncology.
Exceptional
communication, interpersonal, organizational and follow-through skills.
Knowledge
of and ability to perform age specific and disease specific pharmacokinetic
calculations.
Ability to
compound intravenous admixtures and other sterile products.
