Position Description :
The Case Manager will work collaboratively with DFPS staff to identify eligible referrals, share program information and potential referrals, and coordinate the assignment of referrals when appropriate. The Case Manager will provide a smooth and efficient admission process for children/adolescents and their families into DePelchin Children's Center programs. The Case Manager will work with referred families to provide a range of case management services to children and their families enabling them to deal with identified problems. The goal is to optimize the individual strengths of families, their children, and those associated with them in the most effective and efficient manner possible.
Required Qualifications :
Bachelors degree.
Fluent in Spanish. English required.
Preferred Qualifications :
Two (2) years experience in a primary healthcare or related professional setting.
Experience in case management, crisis intervention, or child welfare.
Knowledge, Skills and Abilities :
Knowledge of Social Work practices.
Cultivate an environment that encourages client-centered care, high performance, and collegiality.
Proficient computer skills, including strong MS Office skills, particularly Excel.
Knowledge of Electronic Healthcare Record systems.
Ability to handle unexpected and varied problematic situations.
Ability to exercise independent judgement, decision-making and problem solving.
Strong organizational skills and attention to detail.
Ability to build camaraderie and teamwork.
Possess excellent communication, interpersonal and customer service skills.
Ability to maintain quality control standards.
Work Conditions :
Environment: Office
Range of Schedule: Mon-Fri, 7:00 am to 8:00 pm; Occasional weekends
Travel: Local (Must have reliable transportation and valid Texas driver license)
