Title: Community Health Nurse

Summary: The Community Health Nurse (CHN) is a highly skilled nurse who provides quality primary health care service to special populations to promote and preserve the health of individuals, families and groups to improve the health of the community. They are an exemplary model of the organization's philosophy of care and acts in accordance with policies and procedures.

The CHN can practice independently using critical thinking, evidenced-based practices, and demonstrates mastery technical skills in the clinical setting. They have a solid understanding of community health nursing competencies and can promptly identify participant care needs by utilizing past experiences to implement evidenced-based solutions to address acute issues. This CHN communicates effectively and utilizes community resources for optimal care coordination.

A CHN actively participates in quality improvement projects and collaborates with health care team to support outcome measures. Supports with precepting and onboarding nurses and nursing students based on clinical lead area and daily clinical workflows and operations. Individual work volume will be established through the development of annual Success Objectives, within the framework outlined below.

Essential Duties and Responsibilities:

Clinical Duties:

* Staffs daily RN triage appointments to support acute care needs and chronic disease management education, treatments, and assessment of participants across the lifespan from pediatric to geriatric participants.

* Monitors the RN triage phone line to address participant care needs including: medication refills, lab results, triaging acute symptoms, and care coordination requests as appropriate from participants and/or hospitals, clinics, pharmacies and other external health partners.

* Supports provider team by monitoring desktop of ongoing participant care needs including but not limited to lab reviews, medication education, and wellness checks to promote chronic disease self-management.

* Provide injections for special populations including: PrEP, HIV management, STI treatment, hormone replacement therapy, antipsychotic long acting injectable, Medication Assisted treatments, and family planning.

* Support with health prevention and promotion through vaccine administrations, health screenings, and health prevention and promotion across the lifespan.

* Works collaboratively within a team setting and communicates effectively within clinic team, participants, and family members for continuity of care.

* Supports clinic goals and change initiatives when delivering care.

* Assumes at least one lead clinical task to support clinic's specialty programs and/ or administrative reporting requirements mandated by the Public Health Departments.

* Collaborates effectively with other clinic personnel to ensure task completion is done consistently to optimize participant outcomes.

* Provides education to nursing team or students in ambulatory care setting.

* Accountable for own professional practice, including monitoring and achieving success objectives.

* Models Heartland Alliance's Philosophy of care in clinic practices and demonstrates evidence-based clinic practices and competencies.

Other Job Duties:

* Collaborates with other multidisciplinary professionals and community agencies to provide high quality health care.

* Documents comprehensive, accurate and continual data on client records and program reports

* Practices within the legal and ethical guidelines established by the Nurse Practice Act, the Code of Nurses, the Social Policy Statement and the Standards of Practice for the Community Health Nurse. Adheres to professional standards as outlined by the governmental bodies, American Nurses Associations (ANA).

* Other duties may be assigned.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. * Education and/or Experience: Minimum Associates Degree in Nursing (ADN), Bachelors of Science Degree in Nursing (BSN); with a minimum of at least 3 years of community health experience preferred.

* Experience working with special services including: pediatrics, gender affirming health care, HIV prevention and care, mental health, and/or medication assisted treatment services a plus.

* Licensure: Active Illinois Registered Nurse licensure required.

* Other Qualifications: CPR certification required
