Overview:
Seeking a healthcare professional to help the organization create an environment that supports the mission of the organization to enhance the quality of life for all residents and provide exceptional service to all customers, internal and external.
Key Duties and Responsibilities:
Responsible for the on-going development and day to day operations of the Day Habilitation Center.
Oversees the overall operation of the Day Habilitation program, including but not limited to the following:
Maintain and enforce compliance with agency policy / procedure and all regulatory agencies.
Maintain and facilitate good relationships with state officials, acting in accordance with facility visits and inspections.
Provide oversight, modeling, on the job training and supervision to Direct Care Counselor's (DCC's). Meet with staff regularly to provide necessary feedback. Maintain oversight and development of Supported Employment and Community Inclusion Services.
Ensure all necessary paperwork is completed in accordance with the standards of Medicaid's Comprehensive Waiver (CMW).
Schedule, attend and implement mandatory meetings, trainings, and re-trainings. Ensure training documentation is current and on site in the employee's file or accessible on the agency Network.
Implement supports and services to Individuals in both a group or individual setting, both within the program and the community.
Ensure outcomes and objectives are implemented and documented daily as per the Individuals ISP.
Develop, maintain, and ensure implementation of weekly activity calendars, offering choice based on personal interests and desires of the Individuals receiving services. Order and ensure program supplies are readily available as per weekly activity calendars, to include supplies for alternative activities.
Adhere to and implement ILNJ's Policies & Procedures as well as the employee handbook.
Ensure all program vehicles are operational, maintained regularly, are in good operational condition and equipped with all emergency and compliance equipment and supplies.
Comply with all agencies and / or state inspections and investigations. Follow up on any deficiencies found during internal audits and / or state mandated compliance inspections.
Respond to physical site emergencies appropriately to ensure the health and safety of all Individuals and staff.
Perform other job duties and assignments as delegated by the Program Director or designee.
Requirements
QUALIFICATIONS AND SKILLS: Bachelor's Degree in human service field to include but not limited to social work, psychology, counseling, etc.
At least two years of experience working with the developmentally disabled population
At least four years of supervisory experience
At least five years of an occupational area similar to those being offered in the program
Clearances and Certifications: Due to health and regulatory agency standards, this position is required to be vaccinated and successfully pass background screenings and health clearances.
