Job Description

Department: Clinical Services

Reports To: Clinic Manager

Schedule: Full-time, Monday–Friday day shift

Position Summary

The Care Coordinator supports patient flow, care planning, procedure preparation, and communication between clinical and administrative teams. This role helps ensure patients are prepared, informed, and supported throughout their care journey.

Key Responsibilities

Care Plan & Logistics

Review provider notes for imaging, labs, and follow-up needs

Create and update care plans in the EHR

Monitor outstanding imaging, labs, and diagnostic requirements

Communicate delays or missing items and elevate as needed

Support patients who need clarification about results, care plans, or next steps

Help answer non-diagnostic questions after provider review

Explain procedure expectations, preparation instructions, timelines, and next steps

Document patient questions and follow-up needs clearly

Team Coordination

Review upcoming schedules for missing ROIs, imaging, labs, or documents

Coordinate readiness for upcoming visits and procedures

Communicate updates with the front desk, sales, clinical team, and providers

Support patient flow and assist with rooming as needed

Clinical Support

Room patients and document vital signs

Prepare patients for visits, procedures, and follow-ups

Assist with office-based procedures as needed

Support lab workflows, specimen collection, labeling, and tracking

Maintain exam rooms, supplies, and equipment

Requirements

Graduate of a Medical Assistant or Practical Nursing program

Eligible for CMA certification or LPN licensure within 3 months

BLS/CPR certified or willing to obtain certification

Patient-centered and professional demeanor

Detail-oriented with strong follow-through

Comfortable working in a fast-paced team environment

Preferred Experience

Basic medical terminology

Familiarity with clinical workflows

Minimum 1 year of experience
