Responsibilities:
Lead, inspire, and empower our team of home health nurses to deliver exceptional care that exceeds expectations and enhances the well-being of our patients.
Develop and implement innovative approaches to home health care delivery that prioritize patient-centeredness, autonomy, and holistic well-being.
Foster a culture of compassion, creativity, and collaboration, where every team member feels valued, supported, and motivated to innovate and excel.
Collaborate with interdisciplinary teams, including physicians, therapists, social workers, and community resources, to develop comprehensive care plans that address the unique needs and goals of each patient.
Provide clinical expertise and guidance to nursing staff, ensuring adherence to evidence-based practices and standards of care while promoting a culture of continuous learning and professional development.
Oversee quality assurance and performance improvement initiatives to monitor outcomes, identify areas for enhancement, and drive continuous improvement efforts.
Serve as a liaison between our company, patients, families, and community partners to promote positive relationships, enhance communication, and coordinate care transitions.
Stay current with trends, developments, and advancements in home health care, and incorporate innovative approaches into our nursing practice to enhance patient outcomes and experiences.
Qualifications: Registered Nurse (RN) license with a minimum of 6 of clinical experience in home health care, with 4 of progressive leadership experience.
Strong leadership skills with the ability to inspire, motivate, and mentor teams to achieve goals and exceed expectations.
Exceptional communication and interpersonal skills, with a compassionate and empathetic approach to leadership and patient care.
Strategic thinker with the ability to develop and execute plans to achieve organizational objectives and drive continuous improvement.
Commitment to upholding ethical standards and promoting a culture of integrity, transparency, and respect.
Flexibility, adaptability, and a willingness to embrace new challenges and opportunities with enthusiasm and positivity.
