We’re hiring a Project Coordinator who will drive the implementationof rural health hubs in Reeves, Jeff Davis, and Hockley counties.

This is a home-based position that will require traveling within the three counties mentioned above. The ideal candidate will live in the Texas Permian Basin. This is a grant-funded, temporary, part time (25 hours/week) implementer position for one year. The approximate end date is March 31, 2025.

**Responsibilities**

+ Support community assessments and identify project-related material needs.

+ Coordinate health information distribution with community partners.

+ Function as the primary internal and external contact and resource for the program.

+ Promote and expand positive community relations through active outreach.

+ Recruit, train, and manage volunteers and strategic community alliances to achieve priority community goals.

+ Assist in organizing/implementing community and culturally appropriate health initiatives and programs throughout the three counties.

+ Engage and educate the community with regards to blood pressure resources (coordinate and facilitate market workshops, training, etc. with community, and faith-based organizations) and other health-related initiatives as identified.

+ Regularly evaluate and report on progress towards stated objectives and provide support for achievement of the grant funding deliverables. Facilitate the completion of pre- and post-evaluations.

**Qualifications**

+ Bachelor's degree preferred.

+ 2+ years of experience working with community/public health issues and/or managing community/public health promotion or related educational programs.

+ Bilingual in English and Spanish with the ability to read as well as write preferred.

+ Effective communication, leadership, and interpersonal skills.

+ Knowledge of the social determinants of health (SDOH) as it relates to behavior modification through self-care activities and policy/system-wide changes.

+ Experience working with multicultural & rural communities and in community led programs, projects, or events is preferred.

+ Experience building and working with a volunteer base.

+ Ability to travel within the defined geography and to work evenings and weekends if needed.
