Ritter Center is currently seeking a Program Manager for our Whole Person Care Team . This full-time, exempt position reports to the Chief of Staff. This role is based in San Rafael, CA.

POSITION OVERVIEW:

**This role is based in San Rafael, MARIN COUNTY, NORTHERN CA.**

The Program Manager for the Whole Person Care, also known as Full Service Partnership, Wrap Around Services, or Assertive Community Treatment, team provides client service guidance and staff leadership for a team of case managers serving up to 120 low-income and vulnerable individuals experiencing homelessness and requiring a wide array of mental, medical health, and housing services. This role requires significant experience and knowledge with coordinating and providing services to clients with complex needs while supervising the work of a dynamic team of Case Managers. This position ensures timely and accurate client care data tracking and reporting to the County of Marin in conjunction with Ritter's Quality Improvement staff. Experience with direct-client service is critical to managing this team, although only occasional direct service support will typically be required. This position reports to the Chief of Staff and might involve some Case Manager supervision.

The Program Manager builds, trains, and cultivates an effective team of Case Managers to implement Ritter's wrap-around and highly personalized Whole Person Care service mandate. This role supports the team's development of comprehensive client care plans and the provision of flexible, supportive services to ensure best practices are being used with a Housing First and Harm Reduction approach. Guidance is also provided for the team's efforts to conduct outreach, mental health assessments, screening for eligibility, preparation of reports and clinical case notes for WPC reimbursement. The Program Manager and Lead Case Manager are responsible for developing and maintaining excellent community partner relationships to support clients with access to the variety of mental health, medical care, housing and other entitlement or benefit programs they may qualify for.

Job Duties and Responsibilities:

Recruit, hire, and train new case managers to Housing First and Harm Reduction principles, daily team collaborations, client interventions and visits, shared-case-load principles and functions, and appropriate charting, with the support of the WPC Lead Case Manager

Insure grant deliverables/goals are being met by the WPC team and grant reports are submitted as required by funders

Oversee daily team functions with the support of the WPC Lead Case Manager, such as maintaining accurate casework records and notes, relevant correspondence, and related records and files.

Attend daily Triage and case conference meetings as often as possible to support client care decisions.

Maintain and strengthen existing partnerships with landlords, property managers, and advocates on behalf of individuals served, and collaborate with Marin County Coordinated Entry Team with outreach activities for eligible homeless persons. Create and develop departmental workflows with the WPC Lead Case Manager and Admin Assistant.

Supervise WPC Lead Case Manager, Admin Assistant, and other Case Managers as needed.

Supervise and oversee the implementation of the Enhanced Care Management program for the WPC team, with the support of the WPC Lead Case Manager and Admin Assistant.

Lead WPC team communication and service integration efforts with Ritter's medical, behavioral health, case management/housing, and intensive outpatient treatment programs to support org-wide operational improvements

Attend various Ritter and community partner meetings, often with the WPC Lead Case Manager, to represent the WPC team (MASH - weekly, City Jail Re-Entry - alt Weds, Ritter Leadership - alt Tues)

Verify/approve weekly staff timesheets.

Qualifications: Per OSHA requirements, employment at Ritter Center requires compliance with being able to produce a negative TB test prior to the start of employment, as well as being fully vaccinated against COVID-19 (unless applying for an exemption) per the August 5th State Public Order for Healthcare Facilities, providers and employees.

Bachelor's Degree and/or equivalent experience

Experience coordinating grant-funded programs requiring accurate data collection and reporting, ensuring team accountability to meet grant requirements, detailed contract management, and collaborative decision-making.

Understanding of health care systems and the needs of individuals with complex medical/behavioral health conditions and a history of homelessness.

Demonstrated experience implementing housing/home visit programs guided by the principles of Housing First and Harm Reduction and integrated behavioral health/psychosocial service programs.

Excellent organizational skills and ability to multi-task and remain calm under stress.

Strong leadership and communication skills with the ability to encourage staff.

Creative, out-of-the-box thinking and solutions-oriented approach.

Experience working in a social service setting; ability to be empathetic and compassionate with clients within appropriate boundaries.

In-depth understanding of HIPAA patient privacy protections.

Familiarity with validated clinical assessment tools (PHQ-9, GAD-7, AUDIT-C, DAST, etc.) and Vulnerability Index (VI-SPDAT).

Experience with Motivational Interviewing, Trauma Informed Care, and Patient Centered Care. Experience with the Assertive Community Treatment model is preferred.

Knowledge of Social Services agencies within Marin County and experience maintaining close working relations with partner organizations.

Need to possess a valid and clean CA driver's license.

Experience using computers, MS Office software, and databases is required.

Bilingual in English/Spanish is a plus but not required.
