LSSI is hiring afull-time Manager of Care Managers . Care Manager Supervisor will oversee service delivery of Care Managers, including the coordination of care and services for Williams and Colbert Class Members on the Transition and CST teams. Conduct weekly staff meetings, participate in trainings and conference calls with DHS, UIC-CON, etc. regarding service provision, critical incidents, and complex cases.Ensure overall compliance of services and deliverables for Williams and Colbert Transition and CST teams. Essential Functions: Ensure staff compliance and program compliance, including the timely completion of required documentation.

Ensure compliance with all DHS, Williams, and Colbert Consent Decree standards and deliverables, including overseeing progress toward identified transition targets.

Review documentation to ensure compliance with CCMTP standards/requirements , including EHR documentation.

Train new staff to ensure proficiency in service delivery, comprehension of and compliance with Rule 140 and accreditation and licensure standards, and proficiency with Electronic Health Record (EHR).

Provide direct supervision, training, support, review, and evaluation for assigned staff.

Ensure adherence to ethical standards of treatment, including the NASW Code of Ethics.

Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.

Position Qualifications: Bachelor's degree required with experience. Master's degree preferred.

LCSW/LCPC preferred,

Experience supervising staff.

3-5 years of clinical experience with increasing levels of responsibility preferred.

Proficiency in EHR documentation and review required.

Familiarity with accreditation and licensure standards.

Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.

Excellent organization, presentation and pc/computer skills including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.

Valid driver's license in good standing for the state of residency required.

Access to reliable transportation.Transport self to/from assigned nursing facilities as needed, transport clients to/from appointments as needed, comply with traffic laws/regulations, strong commitment to safety, good navigational skills.

Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
