Summary and Qualifications
The Outpatient Therapist will be using a specialized knowledge of human behavior, enhance problem solving and coping for patients and families throughout the hospital setting. Social Workers provide emotional support, psychosocial assessment, supportive counseling, crisis intervention, group therapy and the provision of information and referrals to appropriate community resources. You will be completing psychosocial assessments on patients referred within two working days. The assessment shall include, but is not limited to assessment of the patient's lifestyle, coping, support systems and needs. Demonstrate a basic understanding of psychosocial issues related to hospitalization for patients of any and all age populations.
You will be participating in interdisciplinary meetings and actively collaborate with treatment staff to coordinate the care of a patient including problem identification, plan of care needs and recommendations to meet the psychosocial needs of the patient.
You will be participate in formulating policies and procedures to facilitate maximum integration of clinical social work services with other disciplines. Demonstrate knowledge of social service policies and procedures. Provide staff with in-service education as needed.
You will be developing relationships with community agencies and serve as liaison to coordinate continuity of care for patients. Plan and coordinate with staff of community facilities, social service agencies, health department and other community agencies and resources to meet patient's post hospitalization needs as appropriate.
You will be providing community and professional staff training regarding patient psychosocial needs and/or coping with illness, grief and loss.
You will be providing evaluation and referral services for patients who are alcohol or drug dependent.
For the Social Worker working in the Behavioral Health Unit shall display knowledge of federal, state and regulatory requirements, (specifically those in the Lanterman-Petris-Short (LPS) Act), plan and conduct individual and group counseling sessions, and provide direct service to patients and their families through coordination of community agency referral/discharge planning. Nonviolent crisis intervention (CPI) training is required on an annual basis.
Schedule: M-F 8:30am - 5:00pm
