Housing Navigator/Community Supports Program Manager

(Southern California)

We are seeking an experienced Program Manager (PM) to oversee and run the day-to-day operations of our Housing Navigator/Community Supports Program in Southern California. This individual will have direct responsibility and oversight for the field team in the identified market. Do you have experience leading people and teams to great success? Job Description:

This position is passionate about the MedZed mission and embodies the STRIVE values daily. The Community Supports PM leads their team and carries a small panel of members. This candidate is focused on ensuring that each member receives the best quality of service and effective member care. This is a remote-field worker role who is primarily out in the community supporting Housing Navigators (HN). Our Housing Navigators Specialize in providing direct social services related to short term and long-term housing. The Program Manager will work closely with the field team to ensure that the goals and objectives which are set for the team are met. The PM supervises staff by articulating expectations, displaying model behavior, maintaining open lines of communication and being clear about roles and responsibilities. A PM's day in the life consists of supporting the team directly in the field, connecting with community leaders and resources, promoting exceptional member care, completing quality audits, and reporting metrics up to the director. A successful PM holds themselves and team accountable to the company, client, and state expectations. PMs are expected to be on call during the scheduled work week and if a team member is in the field. Since the PMs schedule must remain flexible, the PM is expected to flex his/her schedule to meet the program outcomes.

The successful candidate has experience and is comfortable with teaching both remotely and in-person to enhance the skills of our Housing Navigators in the community who serve our members daily. In addition, the candidate possesses strong interpersonal skills with the ability to build and maintain team and community relationships.

Qualifications

Responsibilities:

Maintains minimal Caseload of 15- 22 members.

Manages the day to day for HN Team of 12-18 individuals.

Knowledgeable about all aspects of the HN role

PM identifies open positions on team and collaborates with Director and recruiting to fill positions.

Collaborates with training team to share responsibilities for on boarding new employees and supporting them to ramp up to capacity.

Collaborates with trainer to identify team members who need additional support in the field to improve performance or support HN with a complex member.

Conduct oversight to team through one-on-one performance and caseload reviews, complete field observations and complete required audits.

Ensures HN achieves established performance goals, provides continues coaching and development to the field team.

Participates in correspondence with the health plans, contributes to client calls and collaborating with community leaders.

Completes ride a longs and coaches team members on a variety of needs including but not limited to: Salesforce, Salesforce Maps, New hire support, Care Plan Creation and Panel Management

PM will collaborate with the Director to organize monthly gatherings for the market for education, training review and support team camaraderie.

PM will work with CHNs in the market to obtain feedback and identify topics of education, training, and opportunities for the team to be involved in the community.

Continuously expand knowledge of community resources, services, and programs available to members and build ongoing relationships with these organizations to advocate for members.

Works with Director to develop and implement any SOPs and workflows for program enhancement.

Collaborate with Clinical team to provide one off training on specific refresher & educational aids, i.e., how to find housing resources, benefits offered by Medi-Cal, how to speak with members with SMI or SUD (substance use disorder), motivational interviewing, etc.

Problem solves in a proactive, creative manner.

Able to work with Microsoft Office, Excel, and Outlook (email)

Travel to other markets to spend time with a team in person quarterly or monthly depending on need.

Other Duties as assigned.

Technologically savvy

Qualifications: Bilingual Spanish

Bachelor of Social Work degree, a MSW degree is preferred.

Experience managing people is required.

Minimum of 3-4 years' experience managing others working in health, social, or community services required.

Strong work ethic

Advanced technological and analytical skills

An adaptive person, adaptable to constant change, and working well under pressure in a very fast-paced environment.

Excellent written and verbal communication skills

Candidate Must Possess the Following: Valid Driver's License
