Salary Range: $ 6 0 ,000 – 76,000 Annualized + Full Benefits Package

Caminar is consistently expanding our capacity to address the increasingly complex needs of individuals and families.

Position Summary : The position of Clinical Intake Coordinator is based on a 40-hour work week and is under the supervision of the Clinical Program Manager . The Clinical Intake Coordinator provides compassionate and inclusive care to referred clients. This position is responsible for coordinating admissions with the administrative and programmatic departments within Caminar’s Substance Use Treatment Services. This position will support clients through the agency’s assessment and admission process. This position ensures the appropriate placement of referrals and ensures all requirements are met for network adequacy, including timeliness and access to our services.

Essential Duties and Responsibilities :

Develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with all individuals served.

Work closely with Clinical Lead, Clinical Program Manager, and administrative staff in completing the client intake process and assignment to attending clinician.

Conducts eight to ten intakes per wee k (25% productivity)

Completes documentation promptly showing compliance with agency, county, and department standards.

Ability to navigate and work within an EHR system, to complete documentation, audits, and compliance related to the intake process.

Ability to quickly assess risk related to withdrawal symptoms, suicidal and homicidal ideation, and access supportive resources and referrals to higher levels of care.

Assists in providing ca re coordination services, including bus pass, medi-cal application, recovery residence referrals.

Assists in discharging clients that have disengaged from treatment for 30 days or longer.

Assists in engaging clients in treatment by phone calls and NOABD letters.

Transport clients in company vehicle to local community resources as needed.

Establish and maintain relationships interdepartmentally and within assigned departments to ensure client satisfaction with referrals/admissions/appointments.

Responsible for maintaining a thorough knowledge of community resources, state administrative rules and statutes.

Understands and abides by HIPAA (Health Insurance Portability and Accountability) and confidentiality laws and ethics.

Participate in staff and supervision meetings and attend other meetings and training, as assigned.

Actively nurture and advance the cooperative, harmonious and teamwork-oriented environment Caminar strives to promote within the workplace; through daily efforts and presentation promote an atmosphere of dignity and respect in line with the organization’s mission, philosophy, policies, and procedures.

Will carry a limited case load , provide individual , and group services as needed when clinicians need coverage.

Perform other related duties, responsibilities and special projects as assigned.

May provide occasional front desk evening coverage.

Other duties may be assigned as needed.

Experience, Qualifications, Skills & Abilities:

Must be passionate about Caminar's mission.

Must be registered with CCAPP as RADT or CADC.

Bachelor’s Degree in related field preferred.

Associate degree or equivalent amount of related experience.

Applicant must hold a current California driver's license (Required).

Experience – 2 years of experience working with MediCal programming (preferred).

Experience working in a County System of Care and knowledge of Santa Clara County local resources is preferred.

Thorough understanding of the delivery of services to an ethnically and economically diverse population.

Language Skills – Ability to effectively present information and respond to questions from clients and staff.

Must have excellent computer skills, including extensive work with Microsoft Office (Word, Excel, Publisher, et cetera).

Ability to maintain a high level of confidentiality, a professional demeanor and to represent the organization positively.

Problem solving—identifies and resolves problems in a timely manner and gathers and analyzes information skillfully.

Customer Service—manages difficult customer situations, responds promptly to customer needs, and solicits customer feedback to improve service.

Oral and written communication—speaks clearly and persuasively in positive or negative situations. Completes written progress notes and other formal communications skillfully and professionally.

Quality management—looks for ways to improve and promote quality and demonstrates accuracy and thoroughness.

Must be a dynamic self-starter with demonstrated ability to work independently.

Physical, Environmental and Mental Requirements:

Physical Demands: While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit. The employee is occasionally required to stand, walk and use hands to finger, handle, or feel.

Cognitive: Frequently required to concentrate on moderate detail with constant interruption. Must be able to attend to a task/function for 20-45 minutes at a time. Frequently required to understand and relate to specific ideas, several at a time. Must be able to remember multiple tasks/assignments given to self and others over several days.

Environmental Conditions: Frequent exposure to varied office and mixed (residential/office) environments. Occasional exposure to toxins and poisonous substances, dust, and loud noises.

Equipment: Frequently required to use a computer, phone, and fax machine.

Written Communication - Writes clearly and informatively; Edits work for spelling and grammar; Able to read and interpret written information.

Attendance/Punctuality - Is consistently at work and on time; Ensures work responsibilities are covered when absent; Arrives at meetings and appointments on time.

Special Requirements:

Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Caminar practices.

This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and maintain insurability under agency auto liability policies are all essential job requirements.

Must be able to pass post offer, pre-employment medical and drug tests as required under State Community Care Licensing regulations and/or agency policies.

A personal cell phone with reliable service and, if applicable, a data plan to use for business purposes.

May be required to obtain and maintain First Aid and CPR certification.
