The Care Manager works to achieve daily clinical, quality, and cost outcomes by providing well-coordinated experiences for patients/families through the synchronization of care activities of multiple disciplines and negotiating with third party payors. Able to function in all care management department roles (e.g., TCU Care Manager and Observation Care Manager) after appropriate orientation.This is accomplished by 'negotiating, procuring, and coordinating services and resources needed by patient/family; ensuring and facilitating the achievement of quality; clinical and cost outcomes; intervening at key points for individual patients; addressing and resolving patterns or issues that have a negative quality-cost impact; and creating opportunities and systems to enhance outcomes.' (1)The Care Manager leads the Collaborative Practice Group(s) and related groups. In addition, the Care Manager also maintains activities concerning the CareMaps including the analysis and reporting of variance data. As a part of their outcomes management role, the Care Manager is responsible for daily, weekly and monthly reporting of critical data essential to the operations of the medical center as per the working hours of the Care Management department. QualificationsMinimum Education/Experience:Graduate of NLN accredited School of Nursing.BSN or evidence of enrollment in an accredited BSN program preferredThree years clinical experience required.Clinical expertise in designated case type(s) required.Minimum skills/abilities:Able to communicate verbally and in writing in the English language.Evidence of continues professional development.Possess basic computer knowledge.Ability to identify trends and make connection between care patterns.Possess ability to be a critical thinker.Certification/Licensure:Current licensure in Massachusetts as a Registered Nurse.Current BLS certification required.Evidence of continued professional development. Responsibilities/Essential Functions:Integrates the discharge planning/coordination, utilization review functions in the care management role.Assesses the patient/family for care needs during hospitalization, for discharge and beyond as necessary.Plans the care in collaboration with the health care team for the entire continuum during the hospital stay.Reviews/Evaluates the patient/family plan of care for the individual, as well as for the entire case type(s) along the continuum.Evaluates individual patient's progress intervening and changing the course of care as necessary.Coordinates discharge/post-discharge pletes the roles and functions related to discharge planning/coordination including:Case identificationScreening for high riskAssessing patient and available resourcesGoal setting, planning, implementing, maintaining, evaluating, and documentation corporates knowledge of clinical expertise, quality initiatives, insurance and finance into decision-making and problem-solving of individual patient cases and across case type during care management corporates the highest standard of professional, clinical, legal and ethical practice into their own daily clinical practice (i.e., maintains patient confidentiality and role model to nursing staff).Interfaces, as necessary, with the Admitting, financial counselors, Managed Care, Insurance Case Manager, Finance, and the Screeners.Maintains positive relationships with those mentioned above, as well as representatives of referral agencies and other resources interfaced within our community.Acts as an ambassador, representing St. Elizabeth's to our community.Fosters utilization of facilities within our network.Documents care management activities especially after the initial assessment, as the patient progresses towards discharge, at discharge and after any acute change in the discharge status or circumstance.Consults with the Director of Care Management and/or the VP of Medical Affairs proactively, for the prevention or minimization of patient/family issues.Reports front-line risk management issues in a timely manner to the Director of Risk Managementand the Director of Care Management.Collaborates and participates in risk management issues as deemed appropriate by the Director of Care Management.Effectively utilizes and incorporates data generated from all computer systems for outcomes management i.e., Meditech, TSI, and Q-Works, etc.Identifies and studies trends in individual cases and across case types for the hospital stay, as well as the entire plete the standing departmental reports due on a weekly, or monthly basis (The Volume and Referral), and variance data collection.Develops, collaboratively, interventions for variances in practice, and follow-up by implementing and further evaluating to determine outcomes and best practice.Utilizes critical thinking in everyday care management activities and during outcomes management.Demonstrates effective leadership skills (i.e., health care team, Collaborative Group Practices).Leads, coordinates, and participates in Collaborative Group Practices,( i.e., presents outcome data) when appropriate.Coordinates the development or revision of clinical guidelines, order sets, and CareMaps through the Collaborative Group Practices.Managers/Reports exception data and variance analysis reported from the CareMap.Develops new Collaborative Group Practices or subgroups, as necessary to meet changing needs of clinical practices and for the achievement of desired outcomes.Provides educational/consultation to peers, health team members, Collaborative Group Practices, health care consumers and visitors from other institutions as requested.Precepts Care Management orientates.Practices lifelong learning and is a self-starter.Participates in the Medical Center's Continuous Quality Improvement through ideas generated in daily practice, and a Performance Improvement Project upon approval by the Director of Care Management.Generates presentations to communicate the results of care management projects through story-boards, oral presentation on-site, national speaking and/or publication.Demonstrates advanced critical thinking, decision-making skills, creative problem-solving, and conflict resolution techniques related to patient care issue Steward Health Care is an Equal Employment Opportunity (EEO) employer.
