**Responsibilities**

Coordinate comprehensive plan of care for patients.

+ Oversee primary needs of patient, including assessment of client and family/caregivers, and develop comprehensive plan of care.

+ Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.

+ Monitor patient care through assessments, evaluations and/or patient records.

+ Advocate for resources and removal of barriers.

+ Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

**Requirements**

Licensure / Certification / Registration:

+ BLS Provider preferred. American Heart Association or American Red Cross accepted.

+ Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

+ Counselor preferred.

+ Social Worker preferred.

Education:

+ High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

**Additional Preferences**

+ Counselor or Social Worker with a minimum of 2 years of experience preferred

+ LCSW or LMSW a plus

+ Experience in Hospice, Patient Insurance strongly preferred

+ Ability and willingness to conduct bedside and home health visits as needed (10%-20%)
