Your Role as a Nurse Practitioner/Primary Care Provider:
Empower Seniors: Conduct in-person comprehensive assessments, including initial, semi-annual, and unscheduled visits, to ensure participants receive the care they need.
Craft Personalized Care Plans: Collaborate with the IDT to develop individualized care plans that address each participant's unique needs and goals.
Facilitate Smooth Transitions: Partner with the IDT on discharge planning from acute or long-term care settings, ensuring continuity of care.
Provide Ongoing Care: Make regular visits to participants residing in nursing homes, providing ongoing primary care and monitoring their well-being.
Optimize Care Coordination: Work collaboratively with physicians, community-based PACE physicians, and the IDT to ensure seamless and effective care coordination.
Drive Quality Improvement: Review outcomes and utilization data to identify areas for improvement and implement best practices.
Be Available: Participate in rotating night and weekend call coverage.
Qualifications:
Graduation from an Accredited School of Nursing (for NPs)
Active Nurse Practitioner or Physician Assistant State License in California
Minimum of two years of experience working with the senior population
Current knowledge of chronic care/geriatric issues and best practices
Valid State Driver's License with a clean DMV record
Copy of recent Vehicle Insurance required
