**Nursing Duties:**
The Home Health Nurse coordinates all activities of nursing services in an efficient and caring manner, focusing on quality care and great customer service. Must be available to agency personnel in person or by telephone during the agency's operating hours and in case of emergencies. **In addition, the** Home Health Nurse **will be responsible for the following:**
* Assume the role of agency Administrator when needed
* Be available for state audits
* Help with recruitment, interviewing and onboarding of RNs & LVNs
* Supervise agency pool of RNs & LVNs
* Conduct in-home client assessments and create care plans
* Make onsite visits to perform supervisory and client satisfaction assessments as well as caregiver orientations and training.
* RN delegation of skilled tasks to personal care attendants working with the clients
* Conduct training and educational orientations, in-services & lunch and learns to staff, referral sources and community resources as needed to assist our Community Relations Managers
* Organizing, providing care coordination and care management services to all clients and patients within the agency with a strong focus on those who are most at risk for re-hospitalizations, poor outcomes and those with comorbidities.
**Administrative Duties:**
Assist with office management responsibilities to include helping with:
* **Care Plans & Assessments**
* **Supervisory Visit Reports**
* **Clinical Reports**
* **Advocates, Plans and coordinates the care for seniors to improve their quality of life and to maintain their independence and age safely in place.**
* **Overseeing their client's needs (medical, social and cognitive) and helps the client to successfully navigate the healthcare system.**
**The Ideal Candidate will possess the following qualities:**
* Be able to work in a startup mode environment and help to build a brand from the ground up
* Have strong ethical values and be a loyal person
* Able to work on an on-call, as needed and after hour basis (PRN basis)
* We are seeking a motivated, entrepreneurial and team focused individual. Must be able to work independently, take constructive criticism and hold themselves accountable for meeting activity and sales goals. Passionate for seniors and success. Be goal oriented with an ownership mentality and hungry for success
**Job Requirements:**
* Possess a valid, current, unencumbered license as a Registered Nurse
* Minimum one year of nursing experience in the past 36 months as a Registered Nurse or LVN
* Home Health or private duty experience preferred but not mandatory
* BSN and/or one year of supervisory experience in a healthcare setting a plus
* Must have Valid Drivers License, Auto Insurance, and Reliable Vehicle (and/or be able to drive company car)
* Must be able to pass Drug, Background & Physical Screening
* Copy of updated TB/Chest X-Ray
* Current CPR certification
