Job Description Description: Under supervision of the Nurse Manager, Clinical Director or designee, and in collaboration with other team

members, provides direct and indirect patient care adhering to professional standards of care, state regulations and

accreditation standards and in compliance with Apex Recovery policy and procedure.

Essential Duties and Responsibilities:

Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times.

Assesses client’s status on an ongoing basis and pertinent information gathered is documented.

Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission.

Takes physician’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews physician’s orders. Ensures physician’s orders are legible if taken verbally and transcribed appropriately per policy and procedure.

Accurately administers medications per ordered note and documents accurate count of controlled substance. Adjusts medications under provider’s supervision as needed; monitors lab results to validate medication compliance.

Notifies physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications, etc.

Supervises self-administration of medications using safety initiative of using two means of client identification: documents and reports effects of scheduled and PRN medications.

Maintains accountability of administration, recording, control and security of medications, including narcotics.

Completes admission and discharge processes efficiently and accurately as directed.

Confers regularly with treatment team (e.g., nurses, therapists, program managers, physician) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc.

Provides

education/explanation

to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care.

Monitors client acuity, census, staffing needs, and makes arrangements for coverage as needed, following established guidelines.

Assures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned.

Gives thorough report to oncoming staff, supervisors, and physician.

Standard Expectations

Complies with organizational policies, procedures, performance improvement initiatives and maintains organizational industry policies regarding confidentiality.

Communicate clearly and effectively to the person(s) receiving services and their family members, guests, and other members of the healthcare team.

Develops constructive and cooperative working relationships with others and maintains them over time.

Encourage and build mutual trust, respect and cooperation amongst team members.

Maintains regular and predictable attendance.

Requirements:

Graduate from a state approved LPN/LVN program, and have current and active state licensure

Experience in social services and/or treatment addiction medicine

Valid state driver’s license

Advanced computer skills including Salesforce, Microsoft, Google.

Ability to work professionally with sensitive, proprietary data & information while maintaining confidentiality.

Knowledge of office administration procedures with the ability to operate standard office equipment.

Excellent interpersonal skills including the ability to interact effectively and professionally with individuals at all levels of the organization; both internal and external.

Exercise sound judgment in responding to opportunities; understand when to route opportunities to the next level.

Self-motivated with strong organizational skills and superior attention to detail.

Must be able to manage multiple tasks/projects simultaneously within inflexible time frames. Ability to adapt to frequent priority changes.

Capable of working within established policies, procedures and practices prescribed by the organization.

English sufficient to provide and

receive/instructions/directions.

All other projects and duties as assigned

Physical Requirements:

Move throughout the residential houses and community.

Repetitive hand movement, use and view PC. Use fax, telephone, and copier.

Sits or stand for long periods of time, reach, bend, climb, stoop, and ability to lift up to 25lbs.

Noise levels are consistent with a healthcare or treatment facility

Consistent with the Americans with Disabilities Act, Apex Recovery will provide reasonable accommodations
