Responsible for assigned nursing duties for the Personal Care Management Program. Will Perform clinical screenings to determine clinical eligibility and all duties required under the PCM Program.
Complete initial nursing assessment, screening, and evaluation of potential consumers in the PCM program.
Conducts evaluations and re-evaluations that accurately reflect the consumer’s functional abilities and need for physical assistance in performing ADLs and IADLs
Maintains good working relationships and open communication with doctors and other professionals in the community in order to monitor health status of each participant.
Requires valid driver’s license and reliable transportation.
Education and Experience
Graduate from a school of nursing school accredited by the National League for Nursing
A Bachelor of Science degree in Nursing or an Associates of Science Nursing degree in Nursing and at least one (1) year clinical experience and/or nursing in the community or long-term care setting.
Or
A diploma RN and two (2) additional years of community-based nursing experience or long-term care direct service experience.
Core Competencies
Ability to utilize technology in the field to complete On Site Assessments including Laptops, Tablets, Personal Hot Spots Devices, WIFI, etc.
Working knowledge of spreadsheets, word processing and data entry, including Windows, Word, and Excel helpful
Ability to conduct professional assessments in a variety of home settings in a manner that is respectful and supportive to the occupants of the home
Ability to work with persons with diverse backgrounds, skills and abilities
Experience working with disabled persons preferred
