**Standard Hours: Monday - Friday normal daytime business hours (n** **o on-call, no weekends and no holidays required).** **This is a field based position in the greater Washington DC area and you will be in the field 100% of the time going into members homes.**
**Primary Responsibilities:**
+ Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care in their home
+ Manage members through an acute medical situation and work with chronic conditions, including testing, diagnostics, interventions
+ Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services
+ Manage the care plan throughout the continuum of care
+ Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
+ Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

**Required Qualifications:**
+ Associates degree or higher
+ Current unrestricted Registered Nurse license in Washington DC
+ Certified in Basic Life Support
+ 1+ years of experience in post - acute care, such as long-term care
+ 1+ years of experience with using an Electronic Medical Record
+ Valid Driver's License
+ Access to reliable transportation
+ Ability to travel up to 100% to visit members in their homes within Washington DC
**Preferred Qualifications:**
+ 1+ years of experience working with the geriatric population
+ Experience conducting inservices
+ Field based experience going into patients homes
+ Experience creating care plans
+ Case Management experience
