Program Description

The Behavioral Health Intensive Case Management for Older Adults (ICM) is the newest part of Felton Institutes Geriatric Mental Health Model, which offers specialized mental health services to older adults living with severe psychiatric disorders. Services encompass a continuum of care centering the individual's care goals and needs, including recovery oriented treatment services. Clients served experience a range of needs pertaining to aging, risk of homelessness, institutionalization, and harm reduction/substanse use support. Under contract with the Department of Public Health's Behavioral Health Services (BHS), the ICM program provides services city-wide that include extensive outreach to engage clients in care and multi-disciplinary support to reduce risk of individual's psychiatric hospitalization and to live within their community. The program collaborates with the broad spectrum of social service providers to connect, coordinate, and meet an individual's needs. Felton is committed to implementing evidence-based practices and behavioral health research into its community-based mental health services.

Job Description

Under the supervision of the Program Director, individuals in this position participate in a multidisciplinary team, including master's level clinicians, peer specialists, interns, and psychiatric medication providers and serve as the single point of responsibility for enacting client care. Our programs focus on serving older adults, aged 60+, with severe psychiatric disorders and co-occurring disorders, providing comprehensive and integrated treatment services, including mental health and substance abuse treatment, case management, and other service connections. It employs an inter-agency, multidisciplinary approach to provide comprehensive wellness/recovery-based services. Referrals are authorized by the San Francisco Department of Public Health from multiple community providers and partners, and the ICM caseload is capped at 20 clients to allow for the intensive services needed to stabilize individuals who are experience acute symptoms and related risk factors. Once individuals achieve stabilization, clients are referred for lower levels of care, including outpatient services and/or primary care as clinically appropriate. The ICM clinician evaluates needs of clients, and develops comprehensive clinical assessments, treatment plans, and coordinates care among the team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, and engagement strategies. The ICM clinician also maintains accurate detailed records of client contacts through Cal-AIM (Medi-Cal) and Medicare standards as well as county and state regulations. Additional duties as needed.

Program Specific Responsibilities

Works closely with an interdisciplinary team of clinical case managers, psychiatric nurse practitioners, psychiatrist, and community specialists to provide specialty mental health services to older adults

Provides crisis intervention to prevent hospitalization, and if necessary, faciliates involuntary hospitalization.

Performs intakes and assessments for older adults with psychiatric and dual diagnosis disabilities in a Felton clinic, community, and/or in their homes.

Provides individual or group therapy to decrease mental health symptoms and help clients reach recovery goals.

Provides case management services, including evaluations, service plan development, assisting clients in accessing housing, financial, medical and other needed services. Coordinates with other service providers.

Maintains clinical records of each client contact or service, in accordance with mandated standards of Medi-Cal, Medi-Care, and San Franciscos Community Behavioral Health Services

Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.

Answer a 24/7 after-hours crisis phone for approximately one week a month to support clients in crisis after hours (and earn an additional stipend)

Commits to ongoing training, including developing specializations in evidence-based treatments and practices and the geriatric specialization of mental health services, as provided by the Senior Division and Felton Institute.

Other duties and responsibilities that may be assigned from time to time.

Minimum Required Qualifications:

Masters degree or higher in Social Work or other Professional Counseling Progams, include MFTs/APCCs

Registered with active license with BBS, including intern level ASWs, AMFTs, and APCCs

Actively practices cultural humilty and promotes a warm and welcoming environment for culturally and gender diverse individuals

Demonstrated outreach, rapport building, and engagement experience

Reliability: Accountable; maintains focus; punctual; good attendance record; meets deadlines

Integrity and Ethics: Actively models the highest ethical standards; is honest and accountable; maintains confidentiality and appropriate boundaries at all times; handles sensitive information and issues with discretion and tact

A minimum of 2 years of case management and mental health treatment for at-risk older adults, including co-occuring disorders (masters level internship experience considered)

Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care services definitions in addition to skills in assessment, treatment planning, and mental health services pertaining to individuals living with serious mental illness

Excellent communication skills both in written and verbal formats

Knowledgeable about and consultation oriented towards client risks for suicidality/violence, abuse reporting, dual role conflicts, and other ethical/legal obligations as they arise

Solution focused care planning and conceptualization skills within multi disciplinary team and with external agency partners and individuals

Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models

Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5 and other measures including social determinants of health and functional impairment screenings where appropriate

Valid CA Drivers Licence preferred,

Ability to travel SF to meet clients in a variety of community settings including SROs, hospitals, shelters, and other settings required

Ability to work in a moderate to high stress environment and work well under pressure while meeting multiple different types of deadlines

Strong time management skills

Ability to follow oral and written instructions accurately, be detailed oriented, both give and receive quality improvement feedback

Moderate to High Computer skills proficiency in applications for electronic health care records, zoom, box, microsoft/outlook applications, and other technologies as needec for the role

Available to work 9AM to 5PM, Monday through Friday

Openness to learn, consult, and receive constructive feedback

Ability to be a team player with a welcoming attitude
