Base pay will be determined by applicant’s skills and experience. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Responsibilities

Assist the Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered dementia-friendly programs that meet specific needs of all residents.

Provide person-centered programs while engaging residents in meaningful and purposeful dementia care programs, enhancing their quality of life.

Recognize individual abilities and needs, encouraging independence, and honoring resident habits, routines, interests, and preferences.

Support implementation of person-centered programs to meet resident needs and interests.

Assist in reviewing individual resident service plans and complete program documentation as required.

Ensure dementia care programs comply with state, federal, and other regulations and meet Brookdale’s mission, policies, procedures, and standards.

Provide guidance and resources to associates on the importance of programming and engagement for residents’ quality of life.

Coordinate and participate in special events including family socials, marketing/sales, and open houses.

Communicate positively with residents, families, and community professionals about the dementia care program and its benefits.

Assist with meeting departmental quality improvement initiatives, including the Brookdale Excellence Standards Tool (BEST).

Support planning and scheduled programming events outside the community, which may include driving a community motor vehicle.

Assist with volunteer program, including training and supervising volunteers.

Perform personal care, housekeeping, and dietary responsibilities as needed to ensure service quality.

Qualifications

High School Diploma or equivalent required; Associate’s Degree preferred.

Minimum one year of experience with older adults and persons with dementia required; or equivalent combination of education and experience.

Valid driver’s license and a current 3-year driving abstract with no more than one moving violation.

Physical Demands and Working Conditions

Standing, walking, sitting; use hands and fingers to handle or feel.

Reach with hands and arms; climb or balance.

Stoop, kneel, crouch, or crawl.

Talk or hear; lift up to 50 pounds.

Vision required.

Interaction with co-workers, residents, or vendors.

Occasional weekend, evening or night work; on-call as needed.

Possible exposure to communicable diseases, latex, blood-borne pathogens, chemicals, and other hazards.

Requires occasional travel.

Management/Decision Making Makes standard and routine decisions based on detailed guidelines with little independent judgment; problems generally solved with clear guidelines or by reporting to a supervisor.

Knowledge And Skills Basic level of skills; follow written or verbal instructions; basic technical skills for repetitive duties; basic knowledge of the organization; basic PC and word processing skills, preferably in Windows; effective oral and written communication.
