Job Description

Baptist Medical Center in Jacksonville, FL is seeking an Ambulatory Care Coordinator to join our team. This full-time days position is based at Baptist Metro Square.

You will collaborate with patients and families to discuss advance care planning, engage with patients on end-of-life goals, and work with the care team to develop personalized plans. 1-2 years case management and 1-3 years RN hospital experience required; ASN and BLS credentials required.
